The Kaohsiung Home Nursing Care Center of Chang Gung Medical Foundation was established in September 1996. For many years, our home nursing team has dedicated itself to serving and fulfilling the needs of community patients with chronic illnesses and those requiring long-term care. In addition to delivering professional capabilities and compassionate, attentive services, our home care nurses serve as dedicated caregivers and advocates for individuals in need of long-term support.
Under the project "Angelic Heart, Caring Love: Premium Quality Home Nursing Care Services," the center boasts a comprehensive home care team. This team includes physicians, therapists, social workers, pharmacists, and respiratory therapists who collaborate closely to serve homebound patients, providing essential resources and counseling services based on each individual's unique needs. Furthermore, we place great emphasis on establishing a safe home care environment for every case, which encompasses home medical equipment maintenance, infection control, and preventive hazard protection.

Service volume from 2012 to 2013: Home care nursing visits reached 5,782 in 2012 and 5,963 in 2013; physician home visits reached 947 in 2012 and 982 in 2013.
According to the case closure status from 2012 to 2013, the top three reasons for case closure were mortality (41.51%), stable condition (28.44%), and long-term hospitalization (14.05%). This demonstrates that home care nurses consistently maintain high-quality care for managed patients, facilitating early tube removal to restore their activities of daily living (ADLs).
According to the utilization of resources for home care cases from 2012 to 2013, assistance with medication pick-up (45.5% and 42.1%) and assistance with outpatient clinics (31.2% and 23.5%) ranked as the top two services annually. This demonstrates that although home care is a small unit, nurses still actively assist patients in utilizing social resources based on their needs to solve their problems, further enhancing the quality of care.
Service satisfaction for home nursing cases increased year by year from 2012 to 2013, reaching 96% in 2012 and 96.5% in 2013. Among all indicators, satisfaction with the home care nurses' service attitude, executed nursing techniques, and provided medical care was the highest.
