Regular intra-departmental or multidisciplinary team meetings on medical quality and patient safety are held monthly, reviewing cases that include:
A. Mortality Cases: Unexpected deaths or deaths outside the natural course of the disease.
B. Complication Cases: Unexpected complications caused by examinations or procedures, or severe surgical complications (defined as Grade III or higher according to the Annals of Surgery classification, requiring surgical intervention, being life-threatening, or leading to organ failure).
C. Cases Deemed Necessary for Multidisciplinary Review by the Department Chief, Division Head, or Meeting Chair: Such as unplanned returns to the operating room, unplanned readmissions within 14 days for the same or related conditions, emergency department (ED) returns within 72 hours with subsequent admission, ED returns within 3 days of discharge, prolonged hospital stays (length of stay > 30 days, ICU stay > 14 days, or high-cost ultra-long hospitalizations), and post-resuscitation cases.

Mechanism: Patients or caregivers scan a QR code to complete an anonymous cloud-based survey to evaluate coronary artery disease care.
Results: Statistical analysis demonstrates an average satisfaction rate exceeding 80%.
Action Plan: Substandard items (falling below benchmarks) are formally reviewed in multidisciplinary team meetings to devise corrective and preventive actions.
