The medical center's only home care facility capable of Premature infants simultaneously providing long-term care, ventilator care, care for children with multiple disabilities, rare diseases, and palliative care.
We adopt a comprehensive nursing care approach, ensuring a seamless transition from acute to home care. From admission to discharge, each case receives consistent, holistic, and continuous care from the same nurse. With a stable nurse-patient relationship and a nurse familiar with the patient's condition, we can provide immediate treatment at any time, wholeheartedly helping patients and their families adapt to illness, rebuild their lives, or achieve a peaceful end to life.
To provide continuous medical care services, the Home Care Center affiliated with Chang Gung Memorial Hospital was officially established in 1995. It provides continuous care for individual cases and has consistently invested in nursing staff. In 2007, it had seven dedicated home care nurses, the highest number in the country, and served 5,433 individuals. Home care requires a simultaneous assessment of the individual's physical, psychological, spiritual, social, family care and support systems, and home environment safety, providing individualized services based on the individual's problems and needs.
According to the "Reference Data on Six Key Indicators of Home Care Institution Quality" published by the Long-Term Care Professional Association, the readmission rate for home care cases is 6.26% for medical centers, while our unit's readmission rate is only 6.1%, indicating that our service quality is superior to that of medical centers nationwide.
The threshold for pressure ulcer incidence in home care cases at the medical center is 2%, with a prevalence rate of 14.01%. The prevalence rate of pressure ulcers in our unit is 2.1%, and the incidence rate of pressure ulcers is 0%.
Our organization continuously conducts service satisfaction surveys, primarily to understand the satisfaction of our service recipients and their related suggestions. This information serves as a basis for continuous improvement in service quality, ensuring that our services meet the needs of every family member. Because of our dedicated care and concern for patients, families have given us positive feedback and encouragement, leading to a year-on-year increase in service satisfaction. This motivates us to work even harder to provide better service.
