Description

As Taiwan became a super-aged society in 2025, the emergency department (ED) grew into a crucial hub for geriatric care. Leading this shift, Chimei Hospital established the "Geriatric Emergency Integrated Team" in 2016 and founded Asia's first "Department of Geriatric Emergency Medicine" in 2019. The team spans six core dimensions—administration, screening/care transition, training, quality improvement, equipment, and care strategies—to drive cross-disciplinary collaboration. The team features four innovations: pioneering computer-assisted delirium screening/referral protocols, developing an AI system for geriatric influenza prognosis in the ED, originating ED-based discharge planning and home care referrals, and fully implementing the Hospital-at-Home (HaH) model. Over the years, the team has screened over 130,000 geriatric ED patients and diagnosed 985 delirium cases. In the past three years, it successfully referred 1,684 patients with discharge planning needs and enrolled 400 into the HaH program. This model significantly reduces ED revisit, readmission, and mortality rates among older adults, while its achievements have shaped national policy, leading Taiwan's geriatric ED care into a new era.
