This project focuses on the safe management of antithrombotic therapy in surgical patients. A “Perioperative Antithrombotic Medication Evaluation System” was developed through collaboration among the pharmacy department, IT office, and clinical teams. The system provides automated recommendations for drug discontinuation and resumption, with alerts covering preoperative evaluation, anesthesia consultation, postoperative resumption, and outpatient follow-up. Patients can also consult the pharmacy department or use an online platform for assistance. Following implementation, the preoperative screening rate increased from 58.6% to 100%, significantly enhancing medication safety. Standardized guidelines and visual educational materials further streamline workflows and improve patient adherence. This interdisciplinary model effectively balances bleeding and thromboembolism risks while improving clinical efficiency and patient safety.
An evidence-based and institutionally agreed “Perioperative Antithrombotic Medication Assessment System” was developed to provide automated recommendations for perioperative interruption and resumption of antithrombotic therapy.
The system automatically integrates patients’ thromboembolic history and procedure-specific bleeding risk to support physicians in preoperative risk assessment.
