PN is essential when oral or GI intake is inadequate. HPN enables safe home nutrition support after discharge through caregiver training in catheter care and aseptic techniques.
Since 1999, our team has been dedicated to Home Parenteral Nutrition (HPN) care, establishing a multidisciplinary care model that integrates physicians, nurses, dietitians, and pharmacists. Through continuous innovation in education and patient empowerment, we have developed a one-stop HPN service model. By implementing standardized training, individualized care, and a comprehensive transition-of-care program, we have improved the accessibility and safety of HPN, enhanced patients’ nutritional status, reduced complications, and improved survival and quality of life. In recognition of our outstanding outcomes and innovative service model, the team received the SNQ National Quality Award – Gold Award in 2023. In the same year, we established Taiwan’s first Asia-Pacific Home Parenteral Nutrition Training Center to promote inter-institutional and international collaboration, cultivate healthcare professionals, and share Taiwan’s HPN experience globally. Through these efforts, we continue to advance the quality of clinical nutrition care and strengthen Taiwan’s international influence in the field of nutrition support.
Integrated One-Stop HPN Care Pathway: Established a comprehensive Home Parenteral Nutrition (HPN) care pathway, including standardized clinical protocols and nursing education programs to ensure safe and seamless care transitions.
Innovative Educational Tools and Intellectual Property Development: Developed modular educational support tools and implemented innovative, interactive teaching strategies. These innovations have been recognized with utility model and invention patents in Taiwan, as well as a utility model patent in China.
Policy-Driven Transitional Care and Integrated Healthcare: Aligned with national healthcare policies by promoting vertical integration of medical services and achieving seamless transitions from inpatient and outpatient care to home-based HPN care.
National Excellence and International Leadership: Recipient of the 2023 SNQ National Quality Award – Gold Award. Established Taiwan’s first Asia-Pacific Home Parenteral Nutrition Training Center to cultivate healthcare professionals, promote international collaboration, and serve as a regional benchmark for HPN care.
The number of patients receiving Home Parenteral Nutrition (HPN) at our hospital was 81 in 2023, 78 in 2024, and 64 in 2025.
The total HPN patient-days were 6,113 in 2023, 4,614 in 2024, and 4,965 in 2025.
A total of approximately 223 patients received HPN therapy over the three-year period.
• Home Parenteral Nutrition (HPN) Program Discharge Outcomes, 2023–2025 (N = 186)
Death: 78 patients (41.9%)
Transition to enteral nutrition: 41 patients (22.0%)
Disease progression or transition to hospice/palliative care: 35 patients (18.8%)
Transfer to another healthcare facility closer to home: 8 patients (4.3%)
Change in patient preference: 4 patients (2.2%)
Ongoing HPN treatment: 20 patients (10.8%)
• As of May 2026, the team has published 24 peer-reviewed journal articles and presented 26 conference posters.
•Mechanical Complications:
Catheter occlusion: 33 episodes (0.39‰)
Catheter dislodgement: 12 episodes (0.14‰)
Catheter fracture: 3 episodes (0.04‰)
•Metabolic Complications
Hyperbilirubinemia: 405 episodes (3.30‰)
Hyperglycemia: 341 episodes (4.05‰)
Electrolyte imbalance: 165 episodes (4.80‰)
Hypoglycemia: 22 episodes (0.26‰)
Patient Satisfaction: Prior to HPN intervention, patient satisfaction was only 58.1%, reflecting challenges related to learning requirements and caregiving burden. Through continuous education, hands-on training with educational tools, individualized consultation, and timely support provided by the multidisciplinary team, patients and caregivers gained confidence in home care. Patient satisfaction reached 100% from the third month onward.
Clinical Outcomes: HPN significantly improved nutritional status, as demonstrated by a marked increase in prealbumin levels and improved survival outcomes (p <0.05). HPN was also associated with a 45% reduction in mortality risk (HR 0.55; 95% CI: 0.43–0.71; p < .001). These outstanding outcomes have been published in peer-reviewed journals and serve as a benchmark for HPN practice in the field.
