Diabetic patients and their families
Diabetes is a complex chronic disease, and its treatment and control is a long and challenging process that requires the patient's full cooperation, especially in new cases that need guidance. Our hospital has developed an innovative diabetes care plan called the 4P intervention model, which includes four major steps: 1) developing monitoring software using Power BI, 2) assisting doctors in making accurate decisions on precision care, 3) providing a one-stop service package, and 4) strengthening psychological support for patients. This service offers comprehensive medical care for individuals with diabetes, representing an effective precision care model.
Through Power BI, the medical team can monitor diabetes patients' blood sugar levels, medication usage, and lifestyle changes in real-time. These visualized data not only help doctors quickly understand the progress of each patient's condition but also enable targeted health education services based on specific data. However, since diabetes patients need regular follow-up visits for ongoing care, long-term negative experiences with healthcare may lead to frustration and impatience, which can affect their adherence to treatment.
To address these issues, our hospital has established a Diabetes Package Service, allowing patients to complete their treatment easily and improving their willingness to seek medical care. What diabetes patients lack is not just care knowledge but also sustained support for making lifestyle changes. In response, our hospital has added professional counselors and established a Club to provide psychological support, helping patients cope with the psychological pressures and emotional challenges brought on by diabetes, ensuring that each patient receives comprehensive support and care.
Our hospital uses certified medical devices and has established a comprehensive quality and safety management system to ensure accurate and safe clinical care. The ODES 3010 blood glucose meter and CenterVue DRSplus are approved under the Ministry of Health and Welfare’s medical device licensing system. The Premier Hb9210 HbA1c analyzer is certified by the Taiwan Food and Drug Administration (TFDA). These devices are regularly maintained and inspected by the biomedical engineering team and nursing staff. Other tools, including the Rydel-Seiffer tuning fork, Semmes-Weinstein monofilament, and neurological hammer, are routinely cleaned, maintained, and replaced when damaged to ensure reliability and patient safety. The hospital has also obtained ISO 27001 certification, covering information security risk management, policy implementation, incident management, and continuous monitoring. This ensures the protection of patient data and the integrity of information systems. All new staff and equipment operators must complete standardized training before independent practice. Training includes adherence to operational guidelines and correct equipment usage procedures. Competency is assessed by unit supervisors, and existing staff are evaluated annually. If deviations or deficiencies in quality control are identified, retraining is conducted until competency is achieved. These measures ensure compliance with standards and continuous quality improvement.
Following the implementation of the one-stop service model in 2024, service utilization reached 2,417 visits. As of August 2025, it remained high at 2,358 visits, indicating sustained service demand.
The number of enrolled participants was 2,242 in 2022, 2,375 in 2023, and 2,417 in 2024, with 2,358 participants enrolled as of August 2025, demonstrating a consistent upward trend over time.
