Established in May 2006, our Home Health Care Clinic extends the hospital’s reach to strengthen long-term care services following acute medical treatment. We have expanded our services into remote areas to actively fulfill our social responsibility. Guided by "Love and Care," our patient- and caregiver-centered team care model embraces a holistic approach to supporting individuals and their families. By channeling the passion of our entire team, we aim to enhance the accessibility of healthcare services, expand our diverse roles and functions, and deeply root our services within the community. Service area distribution: Serving residents across the 9 coastal townships of Chiayi County, with an active caseload of N = 259.
Referral & Placement sources: Hospital referrals account for 99.5% of all cases. Among them, 84.94% receive home-based care (at-home cases), and 15.06% receive facility-based care (institutional cases).
Service categories: Home Medical Care, Home Nursing Care, Home Hospice Care, and Long-Term Care Professional Services.
•Co-organizing long-term care related training courses and programs.
•Assisting the County Health Bureau with the filming and production of home hospice care promotional videos.
•Clinical education: Provided community medical practice training and internships for 62 trainees over the past three years.

Skin Breakdown Incidence Rate: 1.17% in 2023, 0.62% in 2024, and 0.53% in 2025.
Unplanned Hospitalization Rate: 11.33% in 2023, 9.71% in 2024, and 8.34% in 2025.
Emergency Department (ED) Utilization Rate: 13.2% in 2023, 11.67% in 2024, and 8.9% in 2025.
Caregiver accuracy rate in performing Intermittent Clean Catheterization (ICC) techniques reached 100%.
Primary caregiver detection rate for Stage 1 pressure injuries reached 100%.
