Target Population

Cataract extraction with severe lens capsule complications

Description

In an eye without sufficient zonular or capsular support, intraocular lens (IOL) implantation can be challenging. The solutions include anterior chamber (AC) IOLs, iris-fixated IOLs, and scleral-fixated IOLs. The scleral-fixated IOL implantation is an important method, especially for patients with compromised corneal endothelium, iris defects, or uveitis history. The topic of our SNQ is according to a novel technique developed by the director of Yangming Branch, Taipei City Hospital. The scleral-fixated IOLs can be performed on almost any kind of single-piece acrylic IOLs under the assistance of microforceps. This technique can fix posteriorly dislocated single-piece acrylic IOLs without taking out the IOLs or fix IOLs in aphakic eyes through small incisions. In addition, this technique secures an IOL with a very low incidence of IOL tilt and a low complication rate.

Key Highlights

  • This technique is developed independently in Taipei City Hospital Yangming Branch.

  • Rescue for severe complication during cataract extraction.

  • Micro-incision surgery: incision less than 2.2mm with 23-27G instruments to avoid surgical induced astigmatism

  • Suitable not only for ordinary IOL, but also multifocal IOL, toric IOL, after PKP patients, combining with trabeculectomy.
