This project adopts a cross-sector co-creation approach to address health inequities arising from uneven medical resource distribution and rapid population aging in rural areas. Tainan Municipal Hospital integrates healthcare, long-term care, social welfare, public agencies, private foundations, and sports organizations to establish a standardized and replicable rural health equity model.
Through mobile clinics, home-based care, functional health assessments (ICOPE), elderly health promotion, and institutionalized charity actions, healthcare services are proactively delivered into communities, creating “10-minute health living circles.” This approach significantly reduces access barriers and enhances continuity of care.
By emphasizing standardized workflows, cross-institutional resource sharing, and outcome-based performance indicators, the model balances quality, efficiency, and sustainability, strengthening community resilience and offering a practical blueprint for integrated rural healthcare.
