This service uses BI intelligent hot spot analysis to deeply understand the medication needs of community residents, and the professional pharmacist team personally delivers medicines to the community. Hot spots, reducing the inconvenience of the elderly going to the hospital due to transportation difficulties. Combining the "Remote Medication Care System" and "Sarcopenia Screening" functions, pharmacists can provide medication consultation in real-time, analyze the health conditions of the elderly, ensure that patients take the correct medication, and improve the quality of care.
In terms of medication safety, the introduction of remote pharmaceutical care services extends the scope of medical care from hospitals to the community. The application of BI technology allows pharmacists to quickly analyze medication distribution, strengthen medication safety monitoring, provide a safer and more convenient medication environment for the elderly, and reduce the risk of hospital-acquired infections caused by readmissions.
The service successfully reduced the average medication time for the elderly from 55 minutes to 20 minutes. In the in-depth community services, our hospital increased the slow-acting medication ratio from 62% to 75%, ensuring that the elderly in the community receive comprehensive and convenient pharmaceutical care services.

This case combines BI hot spot analysis and develops an institutional remote pharmaceutical care model, integrating remote technology with vertical and horizontal integration of internal and external teams, successfully assisting community residents in drug risk assessment, effectively safeguarding the drug safety of community residents, and making an oral presentation at the 2023 ISQua Seoul International Conference (ISQUA2023-ABS-2541). In addition, the overall remote pharmaceutical consultation model has also been recognized by the Ministry of Economic Affairs Intellectual Property Bureau with an invention patent
