• As the Nation's first residential long-term care institution affiliated with a medical center, we are positioned as a "care relaying station for patients with ritical, complex, rare, and difficult-to-manage diseases." • Intelligent Care Systems:Smart Care Mattress System、Human-Centric Lighting、Chatbot、Long-Term Care Information System develop (Long-term care operational dashboard、Six-Grid Layout Communication Board、4MS dashboard、Cloud-based medication system、Seamless data integration between clinical medical systems and long-term care management systems.

• Empowerment Care: Provision of diverse assistive technologies to support individualized rehabilitation therapy, self-reliance support training, deglutition and speech therapy ,Cognitive and Physical Maintenance programs designed to maintain activities of daily living (ADLs) and delay the progression of cognitive decline.

• Approval for Establishment: 88 beds, with an occupancy rate of 98.8%.

• Referral sources: Hospitals (67.0%), home/family (18.7%), and other long-term care facilities.

• Age distribution: Patients aged 60 and above account for 97.7%.

• Disability level: 79.3% of residents are classified as severe disability or above.

• Decannulation success rate: 17.6%

• Nasogastric tube removal success rate: 31.4%

• Urinary catheter removal success rate: 61.1%

• Home discharge rate: 10.4%

• Potentially Inappropriate Medication : Decreased from 58.6% to 46.0%.

• Incidence of falls: < 0.2%

• Incidence of infection: 5.2%

• Unplanned hospitalization rate: 7.6%

Overall satisfaction, including "Satisfied" and "Very Satisfied," reached 90%
