The Home Care Center of the National Taiwan University Hospital, Jinshan Branch, is people-oriented and promotes a continuous and integrated home care model. It combines interdisciplinary teams, connects with the North Coast ACP network, and promotes the North Coast Heroes Integration Alliance to provide innovative services such as home care, palliative care, dream fulfillment projects, and care for bereaved families, becoming a model for home care and palliative care.

In line with government policies and upholding the first line of defense for home care in the North Coast, in addition to general home care, we also undertake institutional palliative care, home-based palliative care, community-based palliative care, long-term care 2.0 home care, integrated home medical care projects, and the government-driven pilot program for in-home emergency care.

Community-based home hospice care coverage is higher than the world average; the home mortality rate and the rate of wishful death are higher than the world average.
A pioneering initiative nationwide: the original home care team continuously tracks bereaved families and regularly holds bereaved family care activities, and has been conducting regular follow-up and tracking for 10 consecutive years.
