This project promotes an integrated long-term care model for rural and remote areas with quality as its foundation, focusing on enhancing care accessibility and strengthening local support capacity.
Wofu Home Care Services has long been deeply rooted in New Taipei City and Taipei City, supported by a well-established, systematized quality management framework, including standardized operating procedures (SOPs), interdisciplinary team collaboration, and comprehensive digital management systems.
Building on this foundation, the organization proactively undertakes responsibility for rural long-term care by establishing a day care center in Gongliao District and integrating community-based service centers with home care services to provide one-stop, coordinated care.
Through partnerships for transportation support, real-time case assignment, and progress monitoring mechanisms, service continuity is ensured. This enables older adults in rural areas to receive long-term care services of quality comparable to those in urban settings, realizing an integrated care model that balances safety, quality, efficiency, and social responsibility.
