A collaborative effort involving a professional team of nurses, social workers, physical therapists, and medical professionals reduced the fall rate among nursing home residents from 0.29% to 0.09%.
Standardized procedures for the rehabilitation team's assessment and scheduling were established. 39.39% of residents showed significant improvement in their Activities of Daily Living (ADL) scores, with the average ADL score increasing from 27.6 to 50.9.
Through cross-team collaboration involving nurses, nutritionists, speech therapists, and home care providers, the nasogastric tube removal rate was 1.5%, and for residents undergoing swallowing training, the removal rate reached 10.3%.
Home Return Rate: There is no definition of "return-to-home rate" for nursing home residents in domestic or international literature. However, successful discharge to home is a core value highly valued by Taichung Tzu Chi Nursing Home. To continuously improve this goal, we have established the "Return-to-Home Rate" as a self-monitored quality indicator. The denominator consists of cases assessed by the team prior to admission who meet the recommended return criteria, including post-acute care (PAC) cases, strong family support systems, cases deemed discharge-feasible after care, potential for functional improvement, clear consciousness, and non-government-subsidized beds. The numerator represents residents who undergo functional training at the nursing home and successfully return home.
The family satisfaction questionnaire surveys five aspects, including the institution's environment, daily living care, health care, service attitude, and residents' rights. Established in 2014, it is conducted twice a year. The most satisfactory categories are: service attitude (average score: 91.02%), followed by the institution's environment (average score: 90.12%), and health care (average score: 88.19%).
As of 2025, the overall satisfaction rate was 91.72%; satisfaction with both service content and service attitude reached 93%.
