China Medical University Hospital's Urology Department, led by Director Chi-Ping Huang, has performed Da Vinci radical prostatectomy since February 2012, accumulating over 2,566 cases by May 2024 — among Taiwan's top 5 in Da Vinci surgery volume. This study focuses on locally advanced prostate cancer (Stages IIIB, IIIC, IVA), using the 4th-generation Xi system to perform "radical" Da Vinci prostatectomy with the innovative "Early Salvage Therapy" approach replacing indiscriminate post-op adjuvant therapy. The study enrolled 79 patients with average follow-up exceeding 60 months. Results: 48% of patients avoided post-op adjuvant therapy; 72% in lymph-node-negative patients. Three-year BCR-free rate: 72.1% (international 63.3%); 5-year BCR-free rate: 57.0% (international 51.0%); overall survival: 98.7%; Stage III survival: 87.80%; Stage IV survival: 64.16% (national 55.35%). Median blood loss 50ml (intl 200ml), post-op complications only 3.8% (intl 12.7%), median lymph node retrieval 24 (intl 16).
