Target Population

Disability or dementia cases

Description

Our institution has introduced smart mattresses and electronic fence systems to provide a high-quality care environment for the elderly. Through non-drug auxiliary therapy activities, we can maintain the elderly's ability to live independently and delay the rate of disability and dementia. Establish a long-term care information system to improve the convenience of team members in resident assessment and record-keeping operations; design diversified courses based on residents’ functional levels, interests and preferences, and provide full-team, full-service, all-round services to “enhance cognition, "Delaying Dementia (Ability)" is the ultimate goal, focusing on reversing the four powers, retrieving two memories, and activating motivation, and creates a golden triangle combination care model that combines "medical care, long-term care, and community" to achieve the goal of aging in place.

As dementia care is attracting increasing attention, the institution has set up a multi-functional brain-building area for dementia. With the aim of "replenishing intelligence and improving memory bravely", it pays attention to the physical and mental condition of the elderly and provides cognitive games to activate the brain and maintain concentration. Strength, memory, etc., create a warm and nostalgic environment, and awaken the forgotten memories of the elderly with dementia.

Key Highlights

An interdisciplinary team of physicians, social workers, dietitians, therapists, and pharmacists provides personalized and continuous care through regular assessments, customized activities, and community-based programs. To ensure high-quality care, staff receive ongoing training in dementia care, palliative care, wellness promotion, stress relief, and fitness to strengthen professional skills and well-being. The facility promotes cognitive health through SODA training systems, AI-assisted interactive games, and brain, physical, and social activities that improve response, attention, coordination, and memory abilities. Diverse programs, music, festive events, and community participation enrich residents’ lives, encourage confidence, independence, and active aging. Smart mattresses and electronic safety fencing reduce pressure injuries, improve sleep quality, prevent wandering, and enhance safety. In 2024, smart healthcare monitoring with automatic vital-sign uploads and integrated care systems was introduced to reduce paperwork, improve efficiency, and provide more complete, technology-enabled care.

Service Data

The nursing home passed the Ministry of Health and Welfare accreditation in 2018 and successfully completed annual service evaluations and supervisory assessments through 2024. The institution received the SNQ (Symbol of National Quality) certification for Long-Term Care Services in 2022 and successfully renewed the certification in 2023 and 2024. With support from the Veterans Affairs Council, smart mattress technology was implemented to monitor residents’ bed-exit activities, provide alerts for high fall-risk residents, and remind staff to reposition residents, improving safety and reducing pressure injuries. Research achievements include poster presentations at national and international conferences: “Impact of Activity Participation on Sleep Quality Among Long-Term Care Residents: Application of a Smart Mattress System” (2024); “Reducing the Density of Urinary Tract Infection in the Nursing Home” at the 27th East Asian Forum of Nursing Scholars (2024); and “Evaluation of Smart Care Technology Use Among Long-Term Care Nursing Staff in Yilan” (2025).
