Description

As this hospital is a public hospital within the county, in addition to implementing government health policies, it also bears the responsibility for the medical care and emergency rescue during major disasters for the county residents. We provide a comfortable medical environment and practical healthcare services to meet the physiological, psychological, and mental health needs of the residents. In response to the demands of an aging population, we have developed specialized medical services, ranging from acute, sub-acute, chronic care to long-term care. Over time, we have expanded our innovative services from non-existent to substantial demand, adopting a holistic perspective on case management to provide necessary services, thereby enhancing public trust in the hospital and offering localized medical services.


Key Highlights

The Nursing Home of Miao-Li Hospital has established an integrated medical and nursing care model for older adults with diverse care needs. Supported by hospital resources, the facility provides long-term care, dementia care, palliative care, respite care, and rehabilitation services within a continuous care system. A key feature is the “Residents Stay, Professionals Move” model. Physicians, nurses, rehabilitation therapists, pharmacists, dietitians, and social workers provide services directly within the facility, reducing transportation burdens and improving access to healthcare. To ensure continuity of care, interdisciplinary teams begin assessment and care planning during hospitalization. Residents receive coordinated services immediately after admission, minimizing care interruptions and supporting smooth transitions from hospital to long-term care. Individualized care plans are developed according to each resident’s needs. Programs include cognitive stimulation, rehabilitation, health promotion, and palliative care to enhance quality of life and dignity. Through continuous quality improvement, patient safety monitoring, fall prevention, and a safe environment, the facility promotes healthy aging and provides a safe, comfortable, and resident-centered second home for older adults.
