Healthcare team with a core principles of patient-centered care protocol and approach, has established an integrated system-based heart team to manage patients with acute myocardial infarction (AMI), including both decision-making and clinical management throughout the whole hospital course. Via the teamwork in cross-functional collaboration and standardized protocols across the emergency department, catheterization lab, intensive care unit, general ward, and post-discharge rehabilitation, seamless AMI heart care of rapid response and high quality can be assured. By utilizing a real-time notification system and individualized care plans, interdisciplinary collaboration is enhanced. Nurses play a key role in pre-operative preparation, post-operative rehabilitation, and self-management education, helping to reduce complications and improve health literacy. The introduction of smart and integrated medical platforms and telehealth technologies enables the integration of medical records and data, supporting visualized and automated care processes that enhance overall efficiency and quality. Moving forward, the team will continue to optimize healthcare protocols and extend to other critical care sectors remaining committed to patient-centered care and keep providing safe, efficient, and continuous healthcare services.
