For 59 years, the Tzu Chi Foundation has provided elderly people with reliable support in their senior years. In 2018, it established the 'Long-term Care Development Center,' setting up 24 long-term care institutions and 147 long-term care locations. The Tzu Chi 880 Health Management and Care System offers three major services including: (1) timely professional care; (2) affordable peace of mind; and (3) attentive healthcare. The Tzu Chi Long-term Care Healthcare System is self-developed and has received patent certificate No. M621437 from the Intellectual Property Office of the Ministry of Economic Affairs. It integrates digital capabilities and adaptability to coordinate core cross-team services, including comprehensive long-term care ABC services extending to medical cross-professional disability prevention and medical care. They have established 92 health care stations (physiological data measurement stations) throughout Taiwan, from north to south, managing 2,472,915 visits to date. The follow-up rate for abnormal cases has reached 100% for four consecutive years, providing timely reminders based on personalized big data. It serves as an important bridge for cross-team collaboration and family care, laying a solid foundation for the new era of smart long-term care.
.Real-time Health Monitoring and Early Warning: Continuously monitor elderly people's physiological data, such as blood pressure and blood sugar levels, and immediately issue alerts when abnormal data appears. This helps in timely detection and management of potential health risks, thereby preventing disease progression.
.Integration with Hospital Information System (HIS) for Precise and Timely Medical Decisions: Large amounts of physiological data can provide detailed health records for doctors, helping them make more accurate diagnoses and treatment plans. This data not only assists in personal health management but also provides important data support for public health research.
.Personalized Healthcare Plans: Continuous health monitoring helps elderly people better understand their health conditions, thereby enabling them to take necessary measures to slow down the aging process and maintain better physical function and mental health.
.Economic Benefits of Real-time Monitoring and Early Warning: Reduces chronic disease and hospitalization costs, lowering the overall public health cost burden.
