Gandau Hospital, centered on “smart integration and community inclusion,” has established a comprehensive diabetes shared-care model for regional hospitals. By integrating cloud-based health platforms and telehealth education, it provides real-time data feedback and case management to enhance self-care and follow-up efficiency. Through collaboration with clinics, pharmacies, and long-term care institutions, it strengthens early detection and lifestyle intervention for high-risk populations. This model combines professional care, digital innovation, and community engagement to deliver accessible, continuous, patient-centered diabetes care, advancing health promotion and sustainable care.

  • Implemented smart healthcare technologies to enable data-driven decision-making, improving precision and efficiency of diabetes care.

  • Integrated smart care platforms with remote monitoring devices to provide home-based health management for older adults and patients with chronic diseases.
