Target Population

Target populations include people with chronic mental illness, dementia, functional disability, older adults needing long-term care, family caregivers, and high-risk community-dwelling elders.


Description

Kaohsiung Municipal Kai-Syuan Psychiatric Hospital has developed a comprehensive “one-stop” psychiatric long-term care model built on psychiatric expertise and integrated community care. The model links an A-level care management center, B-level day care services, C-level community care stations, dementia service sites, a dementia care center, psychiatric long-term care sites, and family caregiver support services to form a continuous care network from medical care to long-term support. Target populations include people with chronic mental illness, dementia, functional disability, high-risk older adults, and their family caregivers. Key features include needs assessment, referral and case tracking, resource coordination, multidisciplinary care, community participation, cognitive and physical function promotion, psychoeducation, interpersonal skills training, peer support, caregiver support groups, respite care, nutrition care, and rehabilitation-oriented activities. In recent years, the hospital has also advanced toward smart psychiatric LTC by introducing generative AI, agentic AI, and intelligent care applications to reduce administrative burden, strengthen risk alerts, support data-driven management, and improve service quality and sustainability.
