St.Mary’s L O HAS Day Care Center was established on May 13, 2019. It was designed based on the concept of "home" for a comfortable living space and a warm feeling of
home. St.Mary’s L O HAS Day Care Center is a mixed type unit for the disabled and dementia. It is centered on the elders and provides self support, zero constraint, and personalized rehabilitation care, allowing the elders to choose the lifestyle they want, Enjoy life with ease and live with dignity
With elder centered care, it provides a small scale,family or iented living environment and personalized care service model for disabled or frail elders, meeting the elders'
diverse care needs and improving their self care ability and quality of life. It can also help the elderly integrate into their daily lives acco rding to their unique characteristics,
maintain and strengthen their existing functions in life, help the elderly live a normal life with peace of mind, and slow down the rate of degeneration.
In response to the growing care needs of older adults with physical disabilities and dementia, the institution has proactively developed comprehensive care services tailored to this population. A diversified program framework structured around seven core pillars has been carefully planned and implemented.
These diversified programs aim to delay functional decline, maintain cognitive and physical abilities, and enhance overall quality of life through a wide range of activities and training. The program components include:
1.Music-Based Care; 2.Reminiscence Therapy; 3.Physical Fitness and Exercise Programs; 4.Cognitive Training Programs; 5.Art Therapy; 6.Social and Community Integration; 7.Spiritual and Pastoral Care.
This unit provides day care services for older adults with disability or dementia, offering a spacious and well-lit activity environment, high-quality activity programs, comfortable assistive facilities, and professional care services. The service helps to reduce the caregiving burden on families. Over the past three years, the average monthly attendance rate has exceeded 89%.
A day care center provided services from January 2023 to June 2025, with an average monthly attendance rate of approximately 88% and about 20 service users. Following the implementation of a seven-core-domain multidisciplinary program, outcomes were evaluated using the Ministry of Health and Welfare Care Management Platform. Care managers and case managers conducted annual reassessments, performing pre- and post-analysis of CMS level, ADL, IADL, and frailty indicators. Attendance varied depending on each user’s long-term care assessment level and approved service quota. Some users recorded attendance below 60%, although they still consistently attended the center about three times per week and actively participated in activities. The CMS (Care Needs Assessment Level) is a key government indicator used to evaluate long-term care needs and allocate resources. It is assessed through home visits by local care management centers and includes ADL and IADL functional evaluations. The CMS is classified into eight levels, with higher levels indicating greater care needs. Overall analysis shows that the seven-domain multidisciplinary program had positive effects in maintaining and partially improving functional abilities among older adults. Future efforts will continue to optimize program design to further enhance daily living functions and overall care outcomes, ensuring sustained effectiveness of long-term care services.
The institution monitors the quality of long-term care services through monthly and annual follow-ups. Abnormal events are reported and reviewed at monthly meetings, and corresponding improvement strategies are discussed. The institution also strengthens the promotion and tracking of the effectiveness of its care model to continuously improve and maintain the quality of care. Regarding abnormal event statistics, there were 2 fall incidents in 2023, 1 in 2024, and 1 in 2025; there were 0 infection incidents between 2023 and 2025; the influenza vaccination rate reached 85.2% in 2023, increased to 91% in 2024, and had not been administered in 2025.
The overall satisfaction rate among older adults and their families reached 95%.
