Liou Ying Chi Mei Home Care, located in the northern region of Zengwen River in Tainan City, has been established for 19 years, serving thousands of families in need of long-term care. We are committed to building a friendly community for the elderly to age in place through home care services. By continuously training and enhancing the capabilities of Home Care Nurses, we aim to make them long-term care experts with the ability to manage family care cases. We develop an all-encompassing care management system that integrates diverse team services to provide comprehensive care for cases, offering community-based, all-encompassing, and continuous services. This allows the elderly to receive quality care at home, reducing the stress and burden on caregivers, and achieving the goal of aging in place with peace of mind.
Home care nurses become family care case managers, developing an all-encompassing care management system that integrates diverse team services to provide comprehensive care for cases. This system connects acute care, chronic care, and long-term care into a secure care network, with a care team consisting of doctors, nurses, pharmacists, social workers, nutritionists, physical/occupational therapists, wound care specialists, discharge planning nurses, etc. Based on the case's disease condition and care severity, as well as family care needs, we provide transitional home medical care, intensive home medical care, dementia care team, palliative home care, wound outreach, and long-term care services, working together to maximize the welfare of cases and caregivers, creating a safe and secure care service circle. The home care center's staffing includes: 1 deputy head nurse, 7 nurses, and 1 assistant, totaling 9 people.
