Target Population

  • Health survey for elderly in Yilan City.

  • Physical fitness promotion services for community elderly in Yilan County.


Description

Since 2012, our hospital has been conducting door-to-door visits for seniors aged 65 and above in Yilan City, with the aim of understanding the local health patterns of the elderly through these visits and care. Starting in 2017, we began offering fitness and frailty assessments for seniors in Yilan County, with nutritional evaluations and physician-recommended prescriptions. This allows seniors to understand their physical condition and receive training from community fitness professionals and health education from nutritionists. After six months, they undergo re-assessment.

The assessment-exercise-assessment model includes four key components: promoting elderly health and fitness, enhancing fall prevention, encouraging healthy eating habits, and strengthening social participation. This comprehensive approach is designed to promote physical health services and achieve the goal of healthy aging within the community.

Service Data

  • Yilan City Elderly Visit Completion Rate: From 2012 to 2016, the first round of visits for seniors aged 65 and above in Yilan City was completed, with a total of 4,083 individuals visited, achieving a completion rate of 48.76% (4,083/8,373). The second round of visits is currently ongoing, and from 2017 to 2025/8, a total of 3,127 individuals have been visited, with a completion rate of 28.6%(3,127/10,928).

  • County-wide Community Assessment Completion Rate: From 2017 to 2024, our hospital completed fitness assessments at 86 sites. The assessment completion rate for C-level sites across the county is 59.72% (86/144).


Featured Outcomes

Safety Outcomes

  • Dietitians Deliver Professional Nutrition Services in the Community:

  • From 2019 to 2022, dietitians provided nutrition services at 95 community care centers across the county.

  • A total of 1,884 older adults received nutritional screening and assessment services. 3.Following the intervention:
    3-1.The proportion of older adults with normal nutritional status increased from 87.2% to 88.11% (+0.91 percentage points).
    3-2.The proportion of those at risk of malnutrition decreased from 12.6% to 11.89% (−0.71 percentage points).
    3-3.The proportion of malnourished individuals declined from 0.2% to 0.1% (−0.1 percentage points).

  • The results indicate that community-based nutrition interventions contribute to improving and maintaining the nutritional status of older adults.


Satisfaction

From 2018 to 2023, a satisfaction survey was conducted among participants of the community-based active aging health promotion activities in Yilan City. The overall satisfaction score was 76.7%. Among the survey items, the section regarding whether participants would be willing to pay out-of-pocket for courses showed dissatisfaction from some elderly participants,This indicates that the acceptance of paying for participation in physical fitness promotion activities among seniors is still not widespread.


International Achievements

Taking the handgrip strength data from home visits to community seniors as an example, comparisons with other Asian regions show that the average values obtained in our study (male 25.4, female 15.5) are lower than those reported by Seino et al. in Japan and Kim et al. in South Korea, but similar to the findings from a study by Ong et al. 2017 in Singapore.
