By integrating cloud-based health platforms and telehealth education, it provides real-time data feedback and case management to enhance self-care and follow-up efficiency. Through collaboration with clinics, pharmacies, and long-term care institutions, it strengthens early detection and lifestyle intervention for high-risk populations. This model combines professional care, digital innovation, and community engagement to deliver accessible, continuous, patient-centered diabetes care, advancing health promotion and sustainable care.
