Target Population

  • Targets: Residents(veterans, dependents, the general public), hospice enrollees, and their families.

  • Groups: Terminal Patients with cancer, chronic renal failure, or frailty have palliative care needs.


Description

  • Hsinchu Veterans Home was established in 1953 and collaborates with Taipei VGH Hsinchu Branch to provide a three-stage, five-level integrated hospice shared care service, fulfilling the mission of "Aging in Place and Dignified End-of-Life".

  • Our project is centered on the I-CARE strategies (Dignified End-of-Life, Continuous Communication, Professional Care, Technology Linkage, and Support with Empathy). Through interdisciplinary collaboration between medical, nursing, and social work teams, we have developed a nationwide-first "Smart Hospice Model".

  • We utilize Smart Fiber-optic Pads for real-time vital sign monitoring, AAC (Augmentative and Alternative Communication) tools to overcome interaction barriers for residents with speech impairments, and VR technology to fulfill residents' dreams of "homecoming through virtual travel".

  • Our outcomes demonstrate excellence: the DNR signing rate has reached 51%, significantly exceeding the national average. Furthermore, the achievement rate of preferred place of death remains at a consistent 100%.

  • Through our warm "Rong-Xin Room" and spiritual care focused on the "Four Stages of Life" (Gratitude, Love, Farewell, and Apology), we not only safeguard residents' dignity in their final journey but also help families find closure, achieving the ultimate value of "peace for both the living and the deceased".

This project, centered on the I-CARE strategy, constructs a one-stop hospice shared care service:

  • Dignified End-of-Life: The "Rong-Xin Room" offers a warm space for families, and supervisors drape the national flag over deceased veterans, highlighting the dignity of life.

  • Technological Intelligence: As a national first, we integrate smart fiber-optic pads for vital signs monitoring, AAC tools to overcome aphasia, and VR for immersive "homecoming" dreams.

  • Professional Systematization: Collaborative networks with Taipei VGH provide interdisciplinary training and social media platforms (FB/Line) for immediate guidance.

  • Holistic Spiritualization: Utilizing the "Four Stages of Life" (gratitude, love, farewell, apology) and grief support. DNR signing rate has reached 51%, fulfilling the goal of "peace for both the living and the deceased".

Service Data

  • Hospice Care and Home Visits: From 2018 to 2026, a total of 54 hospice care cases were accepted, and 983 home visits were conducted.

  • Will Signing and Effectiveness: The resident DNR(Do Not Resuscitate)signing rate reached 51%; from 2018 to 2026, a total of 45 people completed the signing of Advance Medical Directives (ADs).

  • Dream Fulfillment and Spiritual Care: A total of 21 dream-fulfillment events were held; In 2015, religious and spiritual care events were held, with 2,758 participants.

  • Smart Technology Applications: A total of 115 people received VR immersive dream fulfillment services; a total of 234 people used AAC (Augmentative and Alternative Communication).

  • Prevention and Health Care: In 2026, the resident influenza vaccination rate was 94.83%; the employee vaccination rate was 95.4%.

  • Service Recipient Size: Currently, the total number of residents is 415; the service team (including medical and administrative personnel) consists of 207 people.

  • Family Support and Aftercare: From 2020 to June 2026, a total of 104 people have been assisted in applying for funeral subsidies.
