From suspected dementia cases to severe dementia cases and their caregivers
From patients referred by various hospital departments and Community Site A, ranging from suspected cases to severe cases, the hospital provides comprehensive intervention services. Considering the challenges faced by dementia patients, such as the inconvenience of visiting multiple departments due to functional decline, the need for caregiver support, and assistance with care provided by foreign caregivers, our team addresses these aspects holistically. Through service delivery, we identify the needs of patients and their families, integrate resources available both within and outside the hospital, and offer fast and convenient solutions tailored to the service recipients.
A comprehensive one-stop service (from diagnosis to matching with care facilities/services and even late-stage assistive device interventions) is fully available at our hospital. This ensures that both elderly individuals and their families experience reduced anxiety and uncertainty when facing dementia, as well as significantly shortened waiting times for necessary services.
Wanqiao Site Enrollment (2019–Aug 2025): Active cases grew to 45, with a cumulative total of 118 case-times. (App. 1)
Activity Engagement (2019–Aug 2025): Attracted a cumulative total of 35,789 participant-times across sites. (App. 2)
Professional Training (2021–Aug 2025): Hosted 13 sessions (including 20-hour care courses), training 654 professionals to expand the dementia workforce. (App. 3)
Public Outreach (2019–Aug 2025): Held 35 awareness events for 2,171 participants, promoting early detection and resource matching. (App. 4)
Shiye Site (Est. Jan 2023): Enrolled 23 cases; 9 active now, while 14 were successfully referred to Yiren Day Care or other LTC resources based on their needs.
Yiren Day Care (Est. Mar 2023): Capacity cap 24; enrolled 33 cases as of Aug 31, 2025, providing 9,091 cumulative service-times.
Luman Day Care (Est. May 2025): Capacity cap 30; enrolled 22 cases as of Aug 31, 2025, providing 468 cumulative service-times.
Satisfaction: Maintained high standards (98.3% in 2021, 99.4% in 2022) and achieved a perfect 100% in 2023 and 2024, with zero negative or mediocre feedback—proving a highly trusted care mechanism.
I. Staff Quality
Training: All staff completed 20-hr dementia care training with certificates to master communication.
In-service Education: Staff receive ≥8 hrs of dementia care and 3 hrs of infection control training annually.
Staffing: Maintain sufficient staffing based on elders' needs for timely care.
II. Service Quality
Customized Care: Tailor plans based on physical/cognitive status. Assist with daily living and provide cognitive stimulation.
Support & Family Care: Provide social activities to reduce anxiety. Maintain family contact and offer caregiver education.
Evaluation & Referral: Conduct annual evaluations. Refer escalating cases to daycares or nursing homes after family discussion.
III. Safety Measures
Environment: Barrier-free space with clear signage, handrails, and access control.
Medication & Emergency: Enforce strict medication management and emergency protocols.
Anti-Lost: Partnered with police for fingerprinting; 100% of elders at the stations are registered.
IV. Quality Monitoring
Review Meetings:
Quarterly: VP Cheng-Han Tsai chairs LTC Promotion Meetings with department heads for quality tracking.
Monthly: Station meetings address elder issues and track KPI progress.
Audits: Hand hygiene and environmental audits are conducted quarterly since 2024.
Improvement: Optimize workflows based on feedback and audit results.
Since 2021 (the 110th year of the Republic of Era), the Dementia Co-Care Center of our hospital has continuously promoted integrated dementia care through quality optimization and process improvement, conducting annual service satisfaction surveys to collect user feedback. The survey results over the past four years indicate the following:
Overall service satisfaction has consistently maintained a high standard. The satisfaction achievement rates for both 2023 and 2024 reached 100%, with zero negative or mediocre ratings, reflecting that the Center has established a consistent and trustworthy care mechanism.
The number of "Highly Satisfied" respondents has grown continuously. Even as the target clientele and scale of services expanded, a high-quality service experience was successfully maintained.
High recognition was achieved across multiple dimensions, including "case manager service attitude," "efficiency in resource connection," and "process explanation and follow-up care," which serve as the key elements driving the continuous improvement of satisfaction levels.
The four-year trend demonstrates that the Center not only possesses stable and premium service capabilities but also exhibits professional strength in expanding care scale without compromising quality, thereby laying a solid foundation for sustainable development and future promotion. (Appendix 5)

Anti-Disappearance Measures: To prevent elders from wandering off or getting lost, we proactively contacted the Chiayi County Police Bureau to invite officers to our service centers to establish a fingerprint database for the elderly. Currently, 100% of the elders across both centers have completed their fingerprint registration. Since the establishment of these two centers, there have been zero reported cases of elders getting lost during service hours. Since the establishment of the Wanqiao Dementia Care Center in 2019 (Year 108 of the Minguo Era) and the Shiye Dementia Care Center in 2023 (Year 112), there have been zero recorded fall incidents and zero cluster infection events within both facilities.
