Target Population

We serve veterans, their dependents, and the general public,focusing primarily on residential care for seniors and nursing care for the elderly.


Description

Through five innovative service models, we realize the goals of "health enhancement & active aging," "disability reduction & successful aging," and "aging in place & dignified end-of-life." Our approach ensures seniors are not merely recipients of care but are empowered to live authentically, reshape their social roles, and reclaim their self-worth.

Featured Outcomes

  • Carrying forward veterans' love: created a "corridor of memories" and "gratitude tree" (yong-en tree) ,and introduce Intergenerational interaction.

  • Non-pharmacological therapies: effectively improved residents' neuropsychiatric symptoms while enhancing cognition and social interaction.

  • Application of smart fiber-optic sensor mats: reduced the number of falls and unplanned hospitalizations.

  • Revitalizing the campus: transformed spaces, beautified the environment, and successfully lobbied for a stop on bus route 83, creating an age-friendly environment.

  • Palliative & hospice care: enrolled 30 cases, conducted 339 home visits, and assisted 20 residents in passing away peacefully at home (aging in place).

  • Promoting self-reliance support & reablement: achieved 'zero restraints' and 'zero bedridden rate,' with outcomes superior to both domestic and international benchmarks.

  • Influenza vaccination rate: reached 98.5%, achieving herd immunity and outperforming domestic and international averages.

  • Respect for mdical autonomy: the DNR signing rate stands at 51%, significantly higher than the national average.

  • Integrating humanistic care with technological assistive device: we assess individual needs to provide diversified care plans—including self-reliance support & reablement, non-pharmacological therapies, AAC systems, and a 24-hour smart rounds system—yielding excellent results.
