Applications must be submitted by a single institution for one service, technology, process, project or product.
Applications must comply with all relevant laws and regulations, and shall not involve false claims, infringement, counterfeiting, or violations of intellectual property rights.
All submitted documents and supporting reports must be true and accurate. Any falsification or misrepresentation will lead to disqualification or revocation of certification; the applicant will be barred from reapplying for three years and will bear full legal liability.
SNQ and the review committee are obliged to maintain confidentiality of application materials. However, all submitted documents, attachments, and physical products will not be returned, regardless of review outcome.
Once the review process has commenced, application fees are non-refundable, regardless of withdrawal or disqualification.
Upon official notification, certified applicants must sign the "SNQ Mark License Agreement" before being formally authorized to use the SNQ mark and its related identifiers.

Log in to the SNQ Application Portal to complete the application form and download the required templates.
Pay the application fee and upload the required materials. Your application is complete once SNQ confirms receipt.
Complete the online form, print it, and stamp it with the institution's official seal and the legal representative's seal (or have it signed by an authorized representative). Then scan it and upload it as a PDF.
Prepare the plan using your category's template and upload it as an unencrypted PDF. The plan may be up to 50 pages; appendices have no page limit. All content must be complete and backed by supporting evidence.
Photos: 3–5 images showing the service, workflow, key features, and team (JPG or PNG, 4:3 landscape, at least 1000 × 750 px, 300 dpi or higher). Do not use collages, slide screenshots, or text overlays. Make sure no patient or resident can be identified, keep color tones consistent, and include a close-up of any equipment.

Hospital, Nursing Care, Long-Term Care, and Public & Community Services categories
SNQ notifies applicants of the review results by formal written letter each December and publishes the certified list on the SNQ official website. Each certified applicant must sign the "SNQ Mark License Agreement" with SNQ before the certified item may officially use the SNQ mark.
After the announcement, SNQ randomly inspects the quality and mark usage of certified items through market channels; any violation of applicable regulations or significant quality defect will result in revoked mark authorization, with full legal responsibility borne by the applicant.
SNQ conducts unscheduled on-site visits or paper reviews of manufacturing facilities; for factories located overseas, alternative measures beyond on-site visits are defined.
Renewal opens each July so certified items can show that their quality is maintained. Renewal applications and all required documents must be submitted at least three months before certification expires (by September 30).
An approved renewal extends the right to use the SNQ mark and to take part in SNQ promotional programs. Renewed certification is valid from January 1 to December 31 of the following year.
Renewal applicants may also enter the current year's National Biotechnology & Medical Care Quality Award under that year's rules.
If no renewal application is submitted by the deadline, the right to use the SNQ mark ends when certification expires, and the item can no longer be renewed.
The renewal fee is NT$20,000, or NT$30,000 if you also enter the current year's National Biotechnology & Medical Care Quality Award. Fees are non-refundable, even if the renewal or award entry is unsuccessful.
